How to use the Doctor is IN

This new feature of TheFilipinoDoctor.com displays a switch panel on your log in page.
On the center of the switch panel, you will find the switches for IN and OUT.

Demystifying Multiple Sclerosis

What makes multiple sclerosis (MS) special that warrants our awareness? As you will find out here, MS has all the characteristics of a mysterious disease worthy of global attention:

·       MS is rare. Much remains unknown about the disease.

·       For now, it has no known cure yet.

·       The causes of MS are complex, adding to the mystery.

·       It can have severe consequences on people’s lives.

What is MS?

MS is a demyelinating disease. This means that the myelin sheath of the brain and spinal cord, which forms the insulation for the efficient transmission of signals, are destroyed. Because of this, the affected parts of the brain and spinal cord “short circuit” and fail to function properly.

The destruction of the myelin sheath is due to an autoimmune reaction. This means that one’s immune system mistakes the myelin sheath as a foreign substance. Thus, the immune system attacks the myelin sheath, leading to inflammation. This inflammation can disrupt the signals traveling along the nerves of the brain and spinal cord.

The term ‘multiple sclerosis’ comes from the fact that multiple parts of the brain and spinal cord are affected, and the inflammation leaves behind scars (sclerosis) on the damaged nerve cells (neurons).

MS is a lifelong condition, where repeated attacks of MS can lead to permanent damage to the brain and spinal cord, leading to serious disability. MS is also known to slightly reduce the average life expectancy of its victims.

As with many autoimmune diseases, MS is around 2 to 3 times more common among women than in men. The disease is most commonly diagnosed in people in their 20s and 30s and is one of the most common causes of disability in younger adults. Other risk factors for MS include smoking, obesity, and certain viruses (e.g., Epstein Barr virus).

Fortunately, MS is rare. Some reports note that only around 8,400 Filipinos suffer from MS, while other sources note that the number could be even less than 3,000. However, the rarity of this disease could also mean that people with MS could face isolation and neglect—which is why everyone needs to become more aware of MS.

Multiple sclerosis, multiple symptoms

The symptoms of MS are wide-ranging, as the symptoms would depend on which parts of the brain or spinal cord are affected. And because MS affects multiple parts, most patients have more than one symptom, especially in later stages, and the combination of possible symptoms is countless. To complicate things, some people's symptoms worsen steadily over time, while for others, the symptoms come and go.

Some of the most common symptoms of MS include the following:

       Fatigue or an overwhelming feeling of tiredness

       Vision problems, including loss of vision, color blindness, eye pain, flashing lights, double vision, and involuntary eye movements

       Speech and swallowing problems

       Abnormal sensations, such as numbness, tingling, or pain sensations on the face, arms, legs, or trunk

       Muscle spasms, muscle weakness, or tremors

       Clumsiness, poor balance, dizziness, or vertigo (a spinning sensation)

       Pain sensations

       Poor memory, slowness of thought, poor attention span, and other thinking problems

       Bowel problems, including constipation and bowel incontinence

       Urinary bladder problems, such as frequent urination, sudden urge to urinate, frequent night-time urination, or recurrent urinary tract infections

       Sexual difficulties, including low libido, among others.

While MS is rare, if you experience two or more of these symptoms, set an appointment with your physician.

Fighting back

People who are suspected of having MS would need to undergo a test called magnetic resonance imaging. This test uses a machine that takes images of the brain and spinal cord to reveal areas of scarring or sclerosis.

For those who, unfortunately, get diagnosed: while there is currently no cure for MS, researchers are actively searching for one. Furthermore, doctors may recommend treatments that may help control the worsening of symptoms and many other treatments are currently being studied.

The treatment of MS is not a one-man show. Aside from a neurologist, other healthcare professionals such as physical therapists, speech therapists, nurses, etc. will work together to help patients cope with MS.


Living with MS

People with MS need to adopt a healthy and active lifestyle to be able to minimize the impact of the disease on their lives. This self-caring lifestyle includes the following:

·       Staying fit through exercise and a healthy diet

·       Cessation of smoking

·       Attention to mental health

·       Prevention of accidents

·       Regular check-ups at least once a year

In terms of diet, people with MS do not need to take a specific diet. However, a diet rich in complex carbohydrates, fruits and vegetables, and adequate protein and good fats can all help alleviate the most common symptoms of fatigue and constipation.

As for the rest of us, it is important to understand that:

·       MS is not contagious.

·       People with MS may suffer from disabilities as well as frustration, depression, and anxiety.

·       MS may take a toll not just on patients, but also on their loved ones.

·       Therefore, we should offer compassion, understanding, respect, and inclusivity to both patients and loved ones.

Preparing for an MRI

Are you scheduled to get an MRI? Here’s what to expect.

MRI machines are large, tube-shaped machines with powerful magnets inside. Because of this, metal in your body can be attracted to the magnet, which may cause harm. Inform your doctor or the technologist if you have any of the following:

·       Joint prostheses

·       Intrauterine device

·       Artificial heart valves

·       Other implants (heart defibrillator, drug pumps, nerve stimulators)

·       Pacemaker

·       Surgical clips, pins, screws, plates, stents, or surgical staples from previous operations

·       Cochlear implants

·       Ammunition (bullets or shells) or any other type of metal fragment

Inform them also if you:

·       Have tattoos

·       Are pregnant or possibly pregnant

·       Have kidney or liver problems

 

On the day of the test, you will be asked to remove all metal items from your body. You will then be asked to lie down on a small platform. This platform will bring you inside a long narrow tube. The technologist will monitor you from another room, and you will be able to communicate via microphones.

This procedure is painless. Try to lie as still as possible during the test, but keep calm and breathe normally. The whole test takes from 15 minutes up to an hour.
